By registering my child/ren for this program, I agree to the following: In consideration for allowing my child to participate in programs and activities at Paradise Evangelical Free Church, I agree to release and hold harmless Paradise Evangelical Free Church, as well as its employees and volunteers, from any and all liability incurred as a result of my child's participation. I give permission to Paradise Evangelical Free Church, as well as its employees and volunteers, to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental challenges, restrictions, or conditions and/or declare the participant to be in good physical and mental health. If necessary, I request that my child be transported to a nearby hospital.
